About Indianola Physical Therapy
21st Century Rehab, P.C. provides physical,
occupational, and speech therapy, sports medicine services, and occupational health services. 21st Century Rehab is certified in pre-work screening, job matching systems, functional capacity evaluations and stay-at-work/return-to-work program.
We also provide therapy services and consultation to skilled nursing facilities. We consult and provide services for several private and public home health agencies.
21st Century Rehab was founded in 1995 by Steve and Michelle Cassabaum and has five outpatient clinics locations in Altoona, Carlisle, Indianola, Madrid and Osceola; and seven hospital locations at Dallas County Hospital (Perry), Franklin General Hospital (Hampton), Greene County Medical Center (Jefferson), Knoxville Hospitals and Clinics, Story County Medical Center (Nevada), Van Diest Hospital (Webster City) and Wayne County Hospital (Corydon). 21st Century Rehab is headquartered in Nevada.
